IMAT Exam 2027 Guide: Study Medicine in Italy for Indian Students (Dates, Seats, Fees)

IMAT Exam 2027 Guide: Study Medicine in Italy for Indian Students (Dates, Seats, Fees)

If you are an NEET aspirant who is tired of the 20+ lakh cutoff race, or a parent worried about the fees of private medical colleges in India, you have probably heard this line from someone: "Italy mein English mein MBBS ho sakta hai." And yes, it is true. Public universities in Italy run 6-year Medicine and Surgery programmes fully in English, and the entry gate is one exam called the IMAT.

In this IMAT Exam 2027 Guide, we will explain everything in simple language: how the exam works, what we know (and do not know yet) about dates, how seats are divided, what you will actually spend, and how to start preparing from today.

Why Indian Students Choose to Study Medicine in Italy

Before the exam details, let us understand why Study Medicine in Italy is becoming popular among Indian families:

  • English-taught degree: You do not need to learn Italian to get admission, although learning basic Italian helps a lot in hospital rotations and daily life.
  • Public university fees are low: Many state universities charge a flat annual tuition based on category, which is far below Indian private college fees. Pavia, for example, lists a 2026-27 non-EU flat rate of roughly €390 to €4,550 per year depending on the bracket.
  • European-standard degree: The degree follows the EU medical education framework, which can open doors to practice and PG in Europe (subject to each country's recognition rules).
  • Merit-based entry: No donation, no management quota. Your IMAT score decides your ranking.

That is why Study in Italy is no longer a niche idea. Every year more Indian students are comparing it with Russia, Georgia, the Philippines and Germany.

What is the IMAT Exam?

The International Medical Admissions Test (IMAT) is a national entrance test for English-taught Medicine and Surgery courses in Italy. It is conducted once a year, usually in September, and is computer-based and held at approved centres, including centres in India. The test covers logical reasoning and general knowledge, biology, chemistry, and physics and maths. Every candidate gets the same paper format, and wrong answers carry negative marking, so guessing blindly is risky.

Think of it as a mix of NEET-level science and an aptitude test. Your NCERT base will help, but you must also practise reasoning and time management.

IMAT Exam Date 2027: What We Know Right Now

Let us be honest here, because a lot of websites confuse students on this point.

The official IMAT exam date 2027 has not been announced yet. The Italian Ministry (MUR) publishes the dates through its yearly decree, and this usually comes after the previous cycle closes.

Here is the pattern based on recent cycles, so you can plan:

Stage What to expect for IMAT 2027 (tentative)
Document preparation (apostille, Declaration of Value, translations) Start by early 2027, finish before June 2027
Registration on Universitaly Usually opens between July and September (the 2026 window was 26 August to 9 September)
IMAT exam Usually September
Rankings and enrolment rounds October onwards, in multiple rounds
Study visa application For the 2027/28 intake, the deadline is 31 October 2027, and some universities set earlier dates

Important: Treat all of the above as a planning guide, not a confirmed schedule. Once the Ministry's decree is out, we will update this page. You should also always cross-check on the official Universitaly portal.

One more confusion to clear: many blogs say "IMAT 2026" or "IMAT 2027" for the same cycle because the academic year is written as 2026/27. If you are joining in 2027/28, your exam will be in September 2027 (expected), and that is what we mean by IMAT 2027 in this guide.

IMAT 2027 Seats for Indian Students: How Are They Divided?

Seats are fixed by the Ministry for the first year of each course, and they are split into two groups: EU / resident in Italy and Non-EU residing abroad (this is where Indian students fall).

The final seat table for the 2027/28 cycle will come with the new decree. Until then, the 2026/27 table gives a good idea of how the quota looks. Some examples of non-EU seats from that cycle:

University City Non-EU seats (2026/27)
UniCamillus Rome 225
Cattolica del Sacro Cuore Rome 70
San Raffaele (UNISR) Milan 64
Messina Messina 61
Vanvitelli Naples 50
Parma Piacenza 50
Politecnica delle Marche Ancona 50
Pavia Pavia 40
Bologna Bologna 20
Milano Milan 15

A few things to understand:

  1. Seats change every year. Do not choose a university only because it had many seats last year.
  2. Some universities are very competitive with few non-EU seats, so cutoffs are high.
  3. Your preference order matters. At registration you select universities in order, and after the window closes this order is locked. Research cutoffs before you register, not during.

IMAT Fees 2027: What Will You Actually Spend?

This is the part every parent asks about, so let us break it down.

1. IMAT registration fee

In the last cycle, the registration contribution was €130 (about Rs. 14,500 to Rs. 14,600 depending on the exchange rate), paid online on Universitaly with an international-enabled card. It is non-refundable. The 2027 amount is not confirmed yet, so check the portal when registration opens.

2. Document costs in India

These are the "hidden" costs that many students forget:

  • Apostille of Class 10 and 12 documents: approx. Rs. 5,000 to Rs. 15,000
  • Certified Italian translation: approx. Rs. 3,000 to Rs. 8,000
  • Declaration of Value (DoV) from the Italian embassy or consulate, as required

The Universitaly pre-enrolment itself is free. If anyone charges you for it, be careful.

3. Tuition after admission

  • Public universities: Flat or income-linked fees, often in the range of a few hundred to a few thousand euros per year.
  • Private universities in the IMAT list: Much higher, usually in the lakhs of rupees per year.

Add living costs (rent, food, transport, health insurance), which vary a lot between a small city and Milan or Rome. Plan your budget city-wise, not country-wise.

Eligibility in Simple Words

  • You should have completed (or be completing) 12 years of schooling, with Physics, Chemistry and Biology as core subjects.
  • You must be at least 17 years old by the end of the calendar year of the exam, as per the Ministry rules.
  • Your documents must be legalised properly (apostille, translation, DoV).
  • English proficiency requirements differ by university, so check each one.

Always confirm the latest rules in the official decree, because small changes happen almost every year.

How to Prepare for IMAT 2027 (A Practical Plan)

If the exam is in September 2027 and you are starting now, you actually have a comfortable runway.

  1. Now to January 2027: Revise NCERT Biology and Chemistry properly. Strengthen Physics and Maths basics. Start reading daily for the general knowledge and reasoning section.
  2. February to May 2027: Begin topic-wise practice. Solve past IMAT papers section by section and keep an error notebook.
  3. June to August 2027: Full-length timed mocks on the computer. Work on negative marking strategy and speed.
  4. Alongside: Complete your documents early. A delayed apostille can ruin an otherwise perfect plan.

One tip from our counsellors: do not copy NEET strategy 100%. IMAT rewards accuracy and reasoning, not just memory.

Common Mistakes Indian Students Make

  • Waiting for the official date before starting document work
  • Registering on the last day, when the portal and test centres get busy
  • Using a prepaid or wallet-type card that fails on the European payment gateway
  • Choosing universities without checking previous cutoffs
  • Trusting agents who promise "guaranteed seat". No one can guarantee an IMAT seat, because it is purely rank-based

Is Study in Italy the Right Choice for You?

Italy is a great option if you are serious about studying, comfortable preparing for a competitive exam, and open to living abroad for 6 years. It may not suit you if you want a no-entrance-test route. The best way to decide is to compare your budget, your NEET and board scores, and your career goal (practice in India, in Europe, or specialisation abroad). Remember that if you plan to practise in India later, you will need to clear the FMGE/NExT requirements as per NMC rules, so factor that in.
